PARENTAL CONSENT & MEDICAL INFORMATION
The Parent/guardian of a participant/player must inform Michael Johnson Football prior to the commencement of the trials of any medical condition known or suspected of the participant/player which may effect the health or well being of the participant. Please use the space below to let us know of any medical conditions.
 
MEDICAL CONSENT
In the event of an injury or medical emergency a qualified member of Michael Johnson Football staff may administer first aid to the participant. Please tick the box to indicate you give your consent for first aid to be administered.
USE OF IMAGES
Michael Johnson Football may wish to take photographic and or video images of the event for future promotional use or to support local press coverage. If you do not wish your child to feature in any photographs, press reports or videos please tick the box to opt out.
TERMS & CONDITIONS
Your Childs place on the trials will be confirmed in via email or telephone. In the event of adverse weather conditions the trials may be cancelled. If your child is awarded a place on the trials this does not guarantee they will be accepted onto the advance training programme.
